      IVES Request Period Ending Dates

      1. Navigate to Forms > Request for Tax Return (4506 / 4506-T / 4506C)

      2. Under IVES Information complete the Name and Address information

      3. Under Fee complete the # of Returns

      IVES Request Period Ending Dates_02

      4. Select a Form from the Type of Transcript dropdown and enter the requested dates for the Bor and Co-Bor
