8112 Completing the Tax Return Requests screen

The Tax Return Request screen allows you to complete sections needed to populate the print forms for the 4506/4506-T/4506-C and the Tax Information Authorization (8821) used for requesting tax return information. These forms can be Generated and Stored for downloading to your system to send out for wet signing and can also be sent out through Zenly's eSign feature.

To complete the Tax Return Requests screen:

1. Select the Processing tab.

2. Use the left menu panel to click Tax Return Requests.

3. Navigate to the appropriate section for the forms you wish to populate.

 

Request for Transcript of Tax Return (4506/4506-T/4506-C)

1. Click the Add button at the top of the section.

2. Use the Tax Form Number dropdown to make your form selection.

3. Use the Tax Request Status dropdown to make a status selection.

4. Select to individual borrower, or if co-borrowers that file jointly, select the borrower set from the Borrower dropdown.

5. Use the Address Type option to select Current or Previous.

a. Current: This option will populate the Address information listed on the Application > Borrower screen for the borrower.

b. Previous: This option will open the Address fields for manual entry.

6. Use the Alternate Name checkboxes to open the NameSSN/Tax ID and EIN fields for manual entry.

Note: If either of the Alternate Name or Alternate Name/Joint Return checkboxes are selected, then only one form of Identity Number can be entered on the screen.

7. Complete the remaining fields and options on the left as applies to the request.

8. Enter the Request Period End Dates to populate the form.

Note: For both the 4506-T and the 4506-C, only the top row is used on the print form.

9. Click the Save & New button to create addition Requests or Save & Close when completed.

10. All Requests will populate the Request for Transcript of Tax Return table.

11. Complete either section Line 5 - Return To or Line 5a - IVES as appropriate for the document(s) being used.

Each form populated on the Request for Transcript of Tax Return table will be available to Generate or send for eSign individually for all versions of the 4506 form.

 

Tax Information Authorization (8821)

Note: If there is a Co-Borrower on the file, the below steps will need to be followed for each borrower to generate the for for each individual.

1. The Name and SSN/Tax ID field will automatically populate with the information entered for any borrower(s) from the Application > Borrower screen.

2. Complete the EIN and Plan Number fields as applicable.

3. Use the available dropdowns to answer the Authorization questions.

4. Click the Add button if any Designees are to be added to the form.

a. Complete the available fields as appropriate for the Designee being added.

b. Check the Send copies of notices and communications box if copies are to be sent to this Designee.

c. Click the Save & New button to add additional Designees or the Save & Close button when finished.

5. In the Tax Matters section use the Add button to enter information on the Type of Tax information is being requested for,

a. Complete the Add Tax Matters window as applicable.

b. Click the Save & New button to add additional Tax Matters or the Save & Close button when finished.
